There are two forms.  One is for the team coach to comment on the performance of the referee.  The second is for the referee to comment on the team coach and the team.  Communication really is a two way street. 

It is intended that people taking the time to complete the forms will do so in a constructive fashion - good reviews are allowed as well as bad ones.  In this way, the data provides developmental information from both sides of the games to help build the best product we can.  Vindictive or derogatory comments are disregarded

The form filled in by the team coaches must be sent to Steve Scott while the one by the referees must be sent to Jim Flynn.  These forms are held in confidence by both people who use the information as possible discussion pieces with coaches and referees (and their coaches).
